LifeStream Services is your one-stop Aging and Disability Resource Center (ADRC). We can help with a variety of services and in navigating through the steps it takes to get you the help you and your family need.
While many of our services are aimed at older adults and people with disabilities, we also provide transportation and information and assistance to people of all ages.

Adult Protective Services: The Adult Protective Services (APS) Program was established to investigate reports and provide intervention and protection to vulnerable adults who are victims of abuse, neglect, or exploitation. APS field investigators operate out of the offices of county prosecutors throughout the state. East Central Indiana's field office is located in Anderson in Madison County. AngelWorx: This program is for volunteers who are matched one-on-one with lonely and isolated individuals. Angels (volunteers) are asked to make contact at least twice a month with their VIP (very important pal). The program includes opportunities for individuals (Angels), groups (AngelCorps), teens (TeenAngels) and young children (LittlestAngels). Visit our volunteer page for more information.
Attendant Care: This service provides a trained, caring attendant to assist a disabled or disoriented client.
Caregiver Support: Our CareGiver program helps those who are taking care of family members as well as providing support for grandparents who are raising their grandchildren. We offer respite care, support groups, resources and other help for caregivers. The program is part of the Indiana CareGivers Alliance. Care Transitions: Our new Care Transitions program helps qualifying Medicare beneficiaries to transition to home-based care once they are released from the hospital. The goal is to provide participants with tools for better self-care for those who have been admitted to the hospital with certain conditions. Care Transitions is a Community-based Care Transitions Program (CCTP) funded by Medicare and Medicaid Services (CMS) under Section 3026 of the Affordable Care Act.
Emergency Response Service: Our emergency response system – known as Companion Service – is a device worn around your neck or kept nearby that can help you alert others if you have emergency needs; it provides 24-hour access to help. Call our Information and Assistance Center for details on pricing and installation.
Guardianship: We have several years' experience in helping those who can no longer take care of their private affairs. Our guardianship and asset management services help people who don't have family or friends to help take care of their well-being. We serve as guardians for more than 30 individuals through a state-supported program. In addition, we offer LifeGuard, a private guardianship program, by a nationally licensed guardianship counselor.
In-Home Care and Options services: Our care managers help clients connect with the care they need and deserve. Our In-Home Care Managers and Options Counselors assess clients' needs, identify funding sources for services and provide our clients' choices of in-home service providers in their area, authorize delivery of service and follow up with clients to make sure they are satisfied and getting the level of care they need. Options Counselors also assist with transition from nursing facilities back into a residential setting and assist with exploring options for care.
Legal Advice: Legal Services of Indiana at (800) 869-0212 provides advice and counsel when older persons and their families are faced with financial or legal concerns. LifeStream Information and Assistance: We are the regional Information & Assistance center for East Central Indiana, and callers can learn about a wide variety of health and human service groups and agencies. Our Information and Assistance service is also the entry point to services. Here, we make sure you get to the department or receive the information you need as quickly as possible. Making Your Voice Heard (Advocacy): We take a leadership role in advocating on behalf of seniors and people with disabilities. You can help, too, through by calling or writing your government officials or by letting us know what concerns you have. To contact East Central Indiana Legislators, see the attached list. Nursing Home Ombudsman:The ombudsman helps in resolving issues involving long-term care facilities. The state program can make sure your disputes are handled appropriately. Call LifeStream at (800) 589-1121 and ask to speak with the ombudsman.
Nutrition Services: We provide lunches on a donation basis (suggested donation of $3.50 for ages 60 and over) at our Senior Cafes in East Central Indiana. Meals are for those age 60 and older, though those younger than 60 can have lunch for $11 a meal. See our list of Senior Cafes to schedule a visit; just call 24 hours in advance for a reservation. Plan your visit around your favorite foods -- see our Cafe Chat newsletter for what we're serving today. We provide more than 60,000 meals a year through our cafe program. We also deliver more than 180,000 meals to our clients' doors weekdays. Homebound individuals ages 60 and older might qualify to receive a meal at home on a donation basis. Respite Care: Arrangements can be made to give caregivers some time away so they can take care of their own needs or take care of errands for the client. Respite care is an important part of the overall care plan.
Senior Care Services: Senior Care Services helps older adults, people with disabilities and their caregivers who are able to privately pay or provide co-pays for services and programs to help when life gets complex. Transportation: The New InterUrban is a rural transportation service open to people of all ages in Blackford, Delaware, Randolph, Jay, Madison and Henry counties. Wellness Activities: LifeStream has programs available to promote health and wellness among seniors in the community. These include the Chronic Disease Self Management Program class through Stanford University. The class is 2 1/2 hours a week for six weeks and teaches participants how to better manage their chronic health conditions. LifeStream also sponsors an annual Aging Well conference in May.
