In-Home Care Management & Options Counseling
In-Home Care Managers and Options Counselors help current and potential clients connect with the care they need and deserve. They assess the individual’s needs, identify funding sources for services, provide choices of in-home service providers, authorize delivery of service and follow up with clients to make sure they are satisfied and getting the level of care they need. Options Counselors also assist with transition from nursing facilities back into a residential setting, and assist with exploring options for care.
LifeStream Information and Assistance
LifeStream’s Information & Assistance Department is the first line of communication with the caller and our agency. Callers who are interested in learning what services or programs they may qualify for will be assessed by an I&A Options Counselor to determine if a home visit is needed. To expedite the process, you may fill out the contact form (for individuals) or referral form (for health professionals referring a client) located at the top of the page.
In addition to being the point of entry for services, we are East Central Indiana’s Aging & Disability Resource Center which means we can provide callers with a variety of information on health and human service groups and agencies. If LifeStream doesn’t offer the service you require, then we can connect you with the organization who does.

The Process of Starting Services:
Are you or someone you know needing services, but don’t know where to begin? Are you frustrated with the current process of finding assistance in maintaining your independence? Below is the step-by-step process to keep in mind:
- Call LifeStream Services. When calling LifeStream, the caller will speak with an Information & Assistance Options Counselor who will determine if a home visit is needed. An Options Counselor will receive this information to assess the needs of the client, and determine how LifeStream may be able to meet those needs. The Options Counselor will then call to schedule a home visit. This process occurs within 1 to 3 business days.
- A home visit is made with the client. During the visit, the client will be asked questions and will choose a provider for the services they will receive. Then, the home visit documentation is completed and a care plan is developed. The care plan is approved and implemented. This process occurs within 2 to 30 business days.
- Services start as soon as possible. A 30 day follow up occurs after services begin to ensure the needs of the client are being met. The case is then transferred to an in-home care manager. Visits are done every 90 days and services are monitored and adjusted as needed on an on-going basis.
State Health Insurance Assistance Program (SHIP) Counselors
The State Health Insurance Assistance Program provides free, impartial health insurance information for people with Medicare. SHIP can help answer questions regarding Medicare, Medicare Supplement Insurance, Medicare Advantage, Medicaid, long-term care insurance, prescription coverage, and low-income assistance.

Our guardianship services help people who don’t have family or friends to help take care of their well-being. We have several years’ experience in helping those who can no longer take care of their private affairs. We serve as guardians for more than 30 individuals through a state-supported program.
