In-Home Care and Options Counseling
Our staff help clients connect with the care they need and deserve. Our In-Home Care Managers and Options Counselors assess clients’ needs, identify funding sources for services and provide our clients’ choices of in-home service providers in their area, authorize delivery of service and follow up with clients to make sure they are satisfied and getting the level of care they need. Options Counselors also assist with transition from nursing facilities back into a residential setting and assist with exploring options for care.
LifeStream Information and Assistance
We are the regional Information & Assistance center for East Central Indiana, and callers can learn about a wide variety of health and human service groups and agencies. Our Information and Assistance service is also the entry point to services. Here, we make sure you get to the department or receive the information you need as quickly as possible. Just call or fill out the contact form online. Agencies can update their information by using our survey questionnaire for new agency. A copy of our Inclusion Exclusion Policy is also available. Senior Care Services
Senior Care Services helps older adults, people with disabilities and their caregivers who are able to privately pay or provide co-pays for services and programs to help when life gets complex. A variety of programs and services are offered based on cost and need. See the accompanying Senior Care Services Brochure for more. Our Provider Partners 2014-2015 businesses and organizations who deliver services based on care plans arranged by our care managers. Our clients choose from the list for which provider they want to deliver a particular service.
Our CareGiver program helps those who are taking care of family members as well as providing support for grandparents who are raising their grandchildren. We offer respite care, information and other help for caregivers. Arrangements can be made to give caregivers some time away so they can take care of their own needs or take care of errands for the client.
State Health Insurance Assistance Program (SHIP) Counselors
LifeStream’s Aging & Disability Resource Center offers SHIP counseling to assist those in need of help navigating the complex world of health insurance including – Medicare, Medicare Supplement Insurance, Medicare Managed Care Plans, Medicaid, long-term financing financing options and prescription coverage.
