Responsible for assessing individuals’ needs providing information and referral services through the Aging & Disability Resource Center, writing and monitoring service plans, and utilizing resources within guidelines. Provide support to options counselors when county volume is high to reduce the amount of time to complete referrals. Work involves information and assistance, options counseling and community relation development. Travel within LifeStream’s twelve (12) counties required.
Qualifications: Bachelor’s degree from four-year college or university in social work, sociology, counseling, psychology, gerontology, valid IN RN license or a BA degree in any field with a minimum of two years full-time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring; or a Master’s degree in a related field may substitute for the required experience. Licensed Social Work is preferred. Options Counselors must meet experience requirements outlined by the Division of Aging for AD & TBI, Medicaid Waivers, SSBG/Title III, CHOICE, Pre Admission Screening.
