I&A Options Counselor – Delaware County

LifeStream is a non-profit organization working to improve the quality of life for people at risk of losing their independence.  Work of an options counselor involves information and assistance, case management, and community relations.

Qualifications:  Bachelor’s degree in the fields of social work, sociology, counseling, psychology, gerontology, or a valid IN RN license, or a BA degree in any field with a minimum of two years of experience working with the elderly or disabled which includes assessment, care plan development and monitoring; or a Master’s degree in a related field may substitute for the required experience.
