Options Counselor for Grant County

Responsible for assessing client needs providing information and referral services.  Writing and monitoring care plans, and utilizing resources within guidelines.  Work involves case management, resource development/management, and community relation development.

Qualifications:  Bachelor’s degree from four-year college or university in social work, sociology, counseling, psychology, gerontology, valid IN RN license or a BA degree in any field with a minimum of two years full-time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring; or a Master’s degree in a related field may substitute for the required experience. Licensed Social Work is preferred.  Options Counselors must meet experience requirements outlined by the Division of Aging for AD & TBI, Medicaid Waivers, SSBG/Title III, CHOICE, Pre Admission Screening.
