Lifestream is now soliciting nutrition provider partners to provide nutrition services with funding under Title III of the Older American’s Act, NSIP, SSBG, Older Hoosier, Choice, and Medicaid Waiver for fiscal year July 1, 2016 to June 30, 2020. Funding is used to provide nutrition service (congregate meal sites, restaurant meal sites, and home delivered meals) to senior citizens and disabled persons in Blackford, Delaware, Grant, Henry, Jay, Madison, and Randolph counties.
The following estimated funding is based upon a planning allocation from the Indiana Family Social Services Administration Division of Aging available for delivery of in-home and community based nutrition services through fee for service contracts with the area agency.

SSBG category is: home delivered meals
Title III-c1 category is: congregate meals
Medicaid Waiver category is: home delivered meals
Title III-c2 category is: home delivered meals
NSIP categories are: congregate meals and home delivered meals
